We have produced a new record card for Mandatory Training items plus updated the Credit Record Card. These changes have been made to simplify the recording of activities required for promotion and to assist the board when assessing criteria compliance with applications for promotion. The new forms can be found here. Please do not continue to use the old credit record cards. Unused credits can be transferred to the new card where applicable.
